Why Does Prophylactic Epidural Blood Patch Fail to Demonstrate Efficacy in Preventing Post–Dural Puncture Headache in Parturients after Dural Puncture?

چکیده
To the Editor:— We read with interest the study by Scavone et al. that demonstrates the absence of efficacy of a prophylactic epidural blood patch after inadvertent dural puncture. We suggest that two factors could have influenced the negative result of this trial. First, inadvertent dural puncture could have been overdiagnosed, namely when loss of resistance to saline was used to locate epidural space. This could explain the lower incidence of post–dural puncture headache and less frequent realization of therapeutic epidural blood patch reported in this study compared with others. Second, 20 ml may not be the adequate blood volume to test a prophylactic epidural blood patch. This volume has tended to increase over time to 20 ml or more, 23 5 ml in a study by Safat-Tisseront et al. The optimal blood volume may be the volume at which pain in the back, buttocks, or legs occurs, which was only achieved for seven patients in the study of Scavone et al. This higher volume may lead to either a larger patch over the dural tear or a significantly higher increase in lumbar and intracranial pressure, leading to reduced cerebral vasodilation. متن کاملEfficacy of a prophylactic epidural blood patch in preventing post dural puncture headache in parturients after inadvertent dural puncture.
BACKGROUND Postdural puncture headache (PDPH) occurs in up to 80% of parturients who experience inadvertent dural puncture during epidural catheter placement. The authors performed a randomized double blind study to assess the effect of prophylactic epidural blood patch on the incidence of PDPH and the need for therapeutic epidural blood patch. METHODS Sixty-four parturients who incurred inad...
متن کاملPost-dural puncture headache: pathogenesis, prevention and treatment.
Spinal anaesthesia developed in the late 1800s with the work of Wynter, Quincke and Corning. However, it was the German surgeon, Karl August Bier in 1898, who probably gave the first spinal anaesthetic. Bier also gained first-hand experience of the disabling headache related to dural puncture. He correctly surmised that the headache was related to excessive loss of cerebrospinal fluid (CSF). In...
متن کاملEffect of prophylactic gabapentin on post-dural puncture headache in patients after cesarean section surgery
Introduction: Post-dural puncture headache (PDPH) is the most common complication of spinal anesthesia. Numerous drugs have been proposed to prevent or treat post spinal headache, but no definitive treatment is still available. Gabapentin has widely been used for prophylaxis or treatment of different types of headache, so in this study, the preventive effect of gabapentin on PDPH after cesarean...
متن کاملStandardizing Management of Post-Dural Puncture Headache in Obstetric Patients: A Literature Review
Post-dural puncture headache continues to be a significant cause of morbidity in parturients. Despite being a common complication faced by many anesthesiologists, there is a lack of consensus regarding its management. Many still use traditionally taught treatments such as strict bed rest and aggressive hydration despite lack of evidence for their usage. Few are using newly tested treatments suc...
